All notes are stored in localStorage (:124–133, 151, 159) with no network requests. The opens a Tally.so survey on install and sets a Tally.so uninstall URL — both are standard developer feedback patterns using a well-known SaaS form tool, not data exfiltration. The ML score of 0.33 is driven entirely by bundled library size (jQuery, Bootstrap, inflating js_exclamation and total_js_size_bytes) and file count, not by any malicious pattern.
