Nudge Care is an in-EHR clinical copilot for doctors using Semble. It runs in a side panel inside app.semble.io and surfaces structured patient context, lab trends, consultation history, and an evidence-aware assistant exactly where you need it — without forcing you to switch tabs or copy-paste data into a separate tool during a live consultation. WHO IT IS FOR Practicing clinicians who use Semble as their primary electronic health record. The extension is designed around how doctors actually work: short pre-clinic prep windows, time-pressured consultations, and follow-up review between appointments. Nudge Care collapses that overhead by bringing the patient's clinical picture into the same screen where you are already documenting and prescribing. WHERE IT RUNS The extension only runs on https://app.semble.io and does not inject into any other website. When you navigate to a patient page in Semble, the side panel mounts on the right edge of the screen. WHAT IT DOES 1. AI patient summary A 3-4 sentence narrative briefing of the active patient. 2. Lab trends Recent biomarker results with reference-range flags and historical trajectories, normalised to standard units. 3. Consultation history A chronological timeline of recent visits with explicit deltas: chief complaint, what changed since the last consultation, the plan, and any concerning trajectory (worsening symptoms, missed follow-up, escalating dosage without resolution, repeat presentations). 4. Documents Imported clinical documents — referral letters, lab reports, imaging — with quick-access download and one-click resync against Semble. 5. Calculations Clinical scores and derived metrics (BMI, cardiovascular risk indices, body composition) computed from the patient's record. 6. Clinical copilot An assistant you can ask anything about the active patient. It analyses blood work, charts trends inline, summarises the latest consultation in the context of prior ones, suggests next steps with citations, and answers follow-up questions. The copilot operates strictly on the active patient and does not aggregate across patients. MULTI-CLINIC SUPPORT Many clinicians work across more than one Semble practice. Nudge Care models this directly: each clinic you belong to has its own Semble API key on file, validated against Semble at save time, stored encrypted on the Nudge Care backend. Switching clinics inside the side panel transparently routes every Semble call through that clinic's key. AUTHENTICATION Sign in to your Nudge Care account from the side panel using Google, Apple, or your Nudge Care email and password. The session persists across page navigations inside Semble, so you only sign in once per device. Signing out clears your session and per-clinic data immediately. GETTING STARTED 1. Install the extension and pin the icon for easy access. 2. Open https://app.semble.io and click the toolbar icon — the side panel opens on the right. 3. Sign in to your Nudge Care account with Google, Apple, or your email and password. 4. Pick the clinic you are signing in for. 5. Paste the Semble API key for that clinic. Invalid keys are rejected immediately. 6. Navigate to a patient and click "Import patient". 7. Use the Copilot tab to ask questions about the active patient. LIMITATIONS Nudge Care is a clinical-decision-support assistant — not a medical device, not a substitute for professional clinical judgement, and not a replacement for the source EHR. AI-generated content is intended to support, not replace, the clinician's review of the underlying record. Always verify important findings against the source data before acting on them.
