Middletown Assistant is a browser extension, incorporated with the Bridge population health management platform that allows for providers to view, interact, or update population and patient level details at the point of care. Bridge users have details and access to specific elements in regard to performance details on benchmark spending, quality compliance scoring, and contract membership rates, as well as inpatient admissions rates for transitional care management follow-up and compliance. The integrated Bridge physician user has specific patient details at the point of care for risk code re-validation, specific quality measure gap closure, and cost of care analysis. The Middletown Assistant extension is exclusive to current clients of the Bridge platform and uses the current build configuration to set up.
